Direct Support Professionals (DSP/R) are responsible for performing direct active supports, ensuring consistency in implementation of established Person-Centered Plans (PCP), various support plans (Inc.
Health, Safety and Behavioral), and following all safety and health-related policies and procedures.
DSP/R's provides direct support to individuals in all aspects of their home lives.
DSP/R's are responsible for providing support to the individuals receiving services, utilizing teachable moments and methods, as instructed by SD Carter Enterprises, to assist the individuals in all aspects of daily home life with the primary goal of integrating them into their community, as appropriate.
DSP/R's are responsible for completion of documentation pertaining to overall support of the individuals and for related duties, as assigned.
This position commonly works in an unsupervised, independent home setting where the ability to problem solve and make appropriate, on the spot decisions is essential.
DUTIES AND RESPONSIBILITIES Provide residential supports and living skills training to individuals.
Services may be provided at their homes or in the community.
Support individuals in leading self-directed lives by collaborating with individuals and others to plan and implement supports that build upon individual strengths, choices and capabilities that lead to inclusion into the community.
Follow and address and assist in the individual's personal care as needed, including dressing, grooming, feeding, toileting, positioning or medical/health needs.
Report evidence of abuse, neglect and other health and safety issues immediately.
Complete incident reports and give to supervisor timely for review.
Develop and maintain an open line of communication for pertinent information about individuals with coworkers, care givers or employers at beginning and end of shifts.
Interact with the individuals and co-workers in a manner, including language and tone of voice, which is consistent with respect for the individual.
Keep supervisor (or assigned designee) informed of any problematic issues or changes with the individual or in the daily operations of SDCE.
Provide skill reinforcement activities in areas that may include socialization, communication, exercise, recreation, interpersonal skills, financial matters, civil rights, and self-determination.
Provide training to individuals to maintain current skills, learn new life skills, improve social skills and/or modify behaviors that would interfere with successful engagement in their community.
Assist with daily living skills such as meal preparation, menu planning, grocery shopping, cooking, cleaning, laundry, and personal shopping.
If applicable, provide transportation of individuals following SDCE transportation policies and procedures Other duties as assigned COMMUNICATION SKILLS Able to read and effectively interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals and follow them accordingly and consistently.
Able to compose well-written routine reports and correspondence.
This includes but is not limited to:
proper spelling, punctuation and grammatically correct sentence structure that express occurrences in a concise and logical manner.
Ability to follow the organizations writing guidelines necessary to appropriately document PCPs, support logs and other necessary documents.
Speaks to individuals in an adult manner, provides corrective feedback in private, intervenes only as much as needed for safe completion of tasks and provides individuals with the same choices available to all adults.
SKILLS AND ABILITIES Strong interpersonal and communication skills are required in order to develop and maintain positive relationships with adults with disabilities and provide a positive environmental setting.
Also requires the ability to exercise courtesy and tact in dealing with the general community.
Ability to complete basic math.
Necessary skills are required to maintain and check program records, to participate in, and carry out training procedures.
Ability to monitor emergency equipment and supplies, and to respond professionally in a variety of emergency situations.
Conduct emergency drills as directed.
Ability and willingness to professionally support individuals with challenging behavior COMPUTER SKILLS/TECHNOLOGY Working knowledge of computers to allow employee to access computer systems and applications to complete time cards, read and respond to email, and complete required on-line training.
Able to use technology that is necessary to perform job responsibilities; use available technology effectively (voicemail, computer, printer, photocopier, fax and telephone, and various types of audio visual equipment).
Knowledge and/or ability to develop competencies involving use of supportive technology and adaptive equipment to support individuals.
